Origin and Development

Developed by Eli Lilly as a single molecule engineered to activate two incretin receptors, glucose-dependent insulinotropic polypeptide (GIP) and glucagon-like peptide-1 (GLP-1). Fatty-acid modification supports once-weekly subcutaneous dosing.

Mechanism as Understood in the Literature

Dual agonist at the GIP and GLP-1 receptors. The rationale for combining the two is that the incretin effect in humans involves both hormones, and co-agonism was hypothesised to produce greater metabolic effect than GLP-1 agonism alone.

Research Status

Extensive. The SURPASS program in type 2 diabetes and the SURMOUNT program in obesity comprise multiple large randomized controlled trials published in major journals and forming the basis of approval.
